Document checklist
Bring the originals and one photocopy of each. Tick them off as you go — this page prints on a single sheet per interment type.
Burial
- Death Certificate (registered) (required) — from Local Civil Registrar (LCRO)Registered at the LCRO of the place of death within 30 days.
- Burial Permit (required) — from City / Municipal Health OfficeThe park must hold this before accepting remains.
- Transfer Permit (if applicable) — from Health office of originWhen the death occurred in another city or municipality.
Inurnment

- Cremation Certificate (required) — from CrematoriumFrom the crematorium that performed the cremation.
Bone Transfer
- Exhumation Permit (required) — from City / Municipal Health OfficeRequired to move bones from their first resting place.
